2) Set up payment plan and make initial payment
(minimal $250 deposit required)
3) Write player's team number and coaches name on all forms
4) Provide 3 headshot photos with player's name on back of each
5) Returning players to LA Premier FC provide copy of birth document
6) New players to LA Premier FC provide original and copy of birth document
